Secondment opportunity: Be a key part of leading system change to reduce long term segregation for people with a learning disability and autistic people
The HOPE(S) model is an ambitious human rights-based approach to working with people in long term segregation, developed from research and clinical practice. In partnership with Mersey Care NHS Foundation Trust, NHS England and NHS Improvement are funding the model through the NHS-led Provider Collaboratives across England.
The clinical model aims to reduce long term segregation with autistic adults, adults with a learning disability and children and young people in hospital settings and create outstanding culture change across services.
To deliver this important programme specialist practitioners: eight consultant practitioners - 8C and eight specialist practitioners - 8B are being recruited on secondment until Nov 2024 from host organisations who have committed to be a part of this progressive change. The post holders will deliver the HOPE(S) quality improvement programme through supporting clinical teams with practice leadership and training both within their own provider and within regional footprints and will develop sustainable skills as cultural leaders to reduce this practice in the future at organisational and system level.
